HIPAA Terms of Service for Axcelsus Medical Group

Axcelsus Medical Group is committed to protecting the privacy and security of our patients' health information in accordance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and other applicable laws. By using our services, you agree to the terms and conditions outlined in this HIPAA Terms of Service.

Use and Disclosure of Protected Health Information (PHI):

- Axcelsus Medical Group will use and disclose your PHI only for purposes of treatment, payment, and healthcare operations, or as otherwise authorized or required by law.
- We will obtain your written authorization for uses and disclosures of PHI not covered by this agreement or permitted by law. You may revoke this authorization at any time, except to the extent that we have already taken action based on it.

Your Rights:

You have the right to:


- Request restrictions on certain uses and disclosures of your PHI, although we are not required to agree to a requested restriction.
- Access, inspect, and obtain a copy of your PHI.
- Request an amendment to your PHI if you believe it is incorrect or incomplete.
- Receive an accounting of disclosures of your PHI.
- Be notified in the event of a breach of your unsecured PHI.

Axcelsus Medical Group is required to:


- Maintain the privacy and security of your PHI.
- Provide you with this notice of our legal duties and privacy practices regarding your PHI.
- Abide by the terms of this HIPAA Terms of Service.
- Notify you if we are unable to agree to a requested restriction on the use or disclosure of your PHI.
- Accommodate reasonable requests to communicate PHI by alternative means or at alternative locations.

Changes to this HIPAA Terms of Service:

Axcelsus Medical Group reserves the right to change the terms of this HIPAA Terms of Service and to make the new provisions effective for all PHI we maintain. If we make significant changes, we will update this document and provide a copy to our patients.

Complaints:

If you believe your privacy rights have been violated, you may file a complaint with Axcelsus Medical Group or with the Secretary of the Department of Health and Human Services. You will not be penalized for filing a complaint.
